Full Service Partnership Mental Health Clinician

We are currently seeking an experienced Field Based Full Service Partnership Mental Health Clinician . The Clinician is responsible for providing community based mental health services to TAY and Adults in our Full Service Partnership (FSP) program. The target focal population of FSP participants consist of individuals with severe and persistent mental illness who have history of and/or are at high risk for homelessness, incarceration and/or institutionalization.

Services consist of assessments, individual and group therapy, and development of a treatment plan to guide the multi-disciplinary team treating the clients mental health needs. Clinicians perform services which are within the scope of their training and experience.

Duties and Responsibilities:

  • Provide intensive, short or long term individual and group therapy both on site and in the field when appropriate
  • Complete bio-psychosocial assessments and develop treatment plans designed to help facilitate clients growth and to assist with clients ability to interact in the community
  • Assessing and monitoring clients condition to ensure successful transition and stabilization in the community
  • Caseload size will be determined based on the type of services provided, location of services, and severity of cases
  • Maintain timely and detailed clinical records of all client interactions, and other required documentation, which conforms to standards of best practice, and Department of Mental Health guidelines
  • Participate in weekly staff meetings and case reviews as necessary
  • Work in a multidisciplinary team to provide quality services to individuals
  • Collaborate with case managers to ensure the team is assisting with appropriate referrals and linkages into the community including housing, drug and alcohol services, medical care, and other services as needed
  • Maintain appropriate registration/licensure with the State of California’s Board of Behavioral Sciences (BBS), including completion of state required continuing education credits, and hours of supervision

Southern California Health & Rehabilitation Program (SCHARP) was founded February 1994 as a non-profit entity for the purpose of providing mental health and rehabilitation services to vulnerable ethnic minority populations in Southern California Los Angeles. The mission of Southern California Health and Rehabilitation Program (SCHARP) is to provide quality mental health and social services to vulnerable populations in South Los Angeles. These populations consist of mentally ill, homeless mentally ill, multi-diagnosed individuals with substance abuse disorder, interface with the criminal justice system. By providing culturally relevant and sensitive services which are designed to improve the quality of their lives, SCHARP will have a significant impact upon the neediest residents of this community.

Requirement:

The requirements listed below are representative of the knowledge, skills, and/or abilities required to perform the job successfully.

  • Required: MFT, MSW, or clinical psychology licensed or waivered, with one to two years of experience providing services to adults with mental illness and/or substance abuse issues in a mental health environment;
  • Spanish Speaking preferred but not required
  • Knowledge of Evidenced Based Practices, Co-Occurring Disorders and/or Recovery Oriented Models of Treatment, and Motivational Interviewing
  • Good oral and written communication skills;
  • Strong written and oral communication skills
  • Must have valid California driver’s license and the availability of a car with adequate insurance.
  • Must have computer skills and literacy are required to use the agency electronic health record. Staff must be able to logon, type, use a mouse and negotiate through various screens or windows to complete daily chart documentation.
  • Knowledge of Los Angeles County Department of Mental Health’s Rehab Option model of service provision and documentation requirements preferred.
  • Must be able to do field work and availability of rotating on call schedule
  • Must be able to pass the Criminal National and Local Background Livescan check
